This exciting opportunity is for a dedicated and motivated Community Staff Nurse to work across Grange and Windermere and Ambleside Community Nursing Teams as part of the Integrated Care Community (ICC’s) covering a 24 hour period including weekends and bank holidays, 7 days a week.
You may also be expected to work across a variety of DN teams in the Community Care Group to maintain service delivery.
Community Nurses work closely with all health and social care disciplinaries to prevent hospital admission, facilitate hospital discharge and promote independence through reablement and management of long term conditions. The role requires excellent communication skills as you will undertake complex nursing assessments and develop and deliver programmes of care with patients, families and carers.
You will need a full UK divers licence and access to a vehicle in order to fulfil this role.
