Successful candidates will be enthusiastic, movitated and willing to undertake training in stroke care in order to provide Gold Standard care.  The post will be rotated throughout Ward 6, taking into account working within the hyper acute stroke unit and frail elderly ward based care. Duties will include looking after patients in the acute stage of their stroke who need a level of care. This will include observation of monitoring system, recognising the changes in the stroke patient and acting on this information. 

Ward 6 team has very experienced CSW’s and you will be expected to work alongside to deliver great care.  

Successful candidates will report to the Ward Manager and Clinical Leads. They will be expected to partake in training surrounding stroke and frail elderly care.  They would need to show professionalism and work within the NMC Code of Conduct.

The Urgent Community Response and Frailty Coordination Service will enable easy access to decision making support to enable management of patients within the Community with support from a range of services to support admission avoidance or where appropriate refer for an admission.

The post holder would work closely with Primary/ Secondary Care, Adult Social Care and Voluntary agencies in the management of this patient group.

The service will:

  • Create a clear and simple route for Primary Care (and other referrers) to access the most appropriate care for their patient that avoids a default route to hospital 
  • Align existing services/teams to deliver equity of service across the health and social care economy 
  • Support the care for Frail Elderly people in an environment that meets with their needs and their wishes 
  • Aim to improve the quality outcomes for patients

Main Duties to include:

– Develop and maintain communication with people about complex issues and/or in difficult situations. Client groups will include people who inject drugs, prisioners, migrant and hard to reach populations.

– Support the Hepatology outpatient team with service delivery, expansion and complying with protocols.
