Discharge Case Manager.  Hospital Discharge Team           

We are looking for applicants who would like to come and work with the discharge team in UHMB.

You must be professional registered with a good level of knowledge and experience within hospital discharge. You must have a track record of successful multiagency working and have knowledge of the pathways available to support patients to leave hospital. The post requires a degree of flexibility and the ability to work autonomously in conjunction with other stakeholders to ensure a safe transition from hospital.

You will be able to demonstrate that you are forward thinking, dynamic and can implement developments to achieve efficient high quality services. The ideal candidate needs to embrace change as we move towards a whole system for discharge planning 7 days a week and be able and willing to travel across multiple sites and to community settings.

We are currently looking at integrating our health discharge teams across UHMB and CPFT and changing the way in which our service operate so please be aware that current job descriptions is subject to change.

 Anyone newly appointed to the Trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.

We are looking for an Admin Assistants to support with the Discharge to Assess (D2A) programme.

D2A Programme Duties:

  • Minute taking & arranging meetings
  • Data input and writing reports (from data/notes)
  • Keeping spreadsheets up to date
  • Maintaining weekly blog/typing up patient case studies
  • Updating and maintaining the intranet page for discharge and D2A
  • Creating presentations from content
  • Organising diaries
  • Support with patient engagement and recording patient experience
  • Completion of action lists and chasing up of these actions
  • Updating medically fit and delayed transfers of care spreadsheets
  • Web research re case studies and best practice

Required Skills: good communicator, self-motivated, excellent IT skills, good telephone manner, administrative skills, able to grasp ideas and develop them, good design skills, ability to liaise with colleagues across health and social care and at different levels in the organisation, ability to work flexibly and support the team.
